Several complexities of the eCB system may impact upon the potential of CBD and other CB1R-activating agents to serve as anxiolytic drugs. First, CB1R agonists, including THC and AEA, have a biphasic effect: low doses are anxiolytic, but higher doses are ineffective or anxiogenic, in both preclinical models in and humans (reviewed in [33, 45]). This biphasic profile may stem from the capacity of CB1R agonists to also activate TRPV1 receptors when administered at a high, but not low dose, as demonstrated for AEA . Activation of TRPV1 receptors is predominantly anxiogenic, and thus a critical balance of eCB levels, determining CB1 versus TRPV1 activation, is proposed to govern emotional behavior [27, 47]. CBD acts as a TRPV1 agonist at high concentrations, potentially by interfering with AEA inactivation . In addition to dose-dependent activation of TRPV1 channels, the anxiogenic versus anxiolytic balance of CB1R agonists also depends on dynamic factors, including environmental stressors [33, 49].
In a series of placebo-controlled studies involving 15 healthy volunteers, Fusar-Poli et al. investigated the effects of CBD and THC on task-related blood-oxygen-level dependent functional magnetic resonance imaging activation, specifically the go/no-go and fearful faces tasks [109, 110]. The go/no-go task measures response inhibition, and is associated with activation of medial prefrontal, dorsolateral prefrontal, and parietal areas . Response activation is diminished in PTSD and other anxiety disorders, and increased activation predicts response to treatment . CBD produced no changes in predicted areas (relative to placebo) but reduced activation in the left insula, superior temporal gyrus, and transverse temporal gyrus. The fearful faces task activates the amygdala, and other medial temporal areas involved in emotion processing, and heightened amygdala response activation has been reported in anxiety disorders, including GAD and PTSD [113, 114]. CBD attenuated blood-oxygen-level dependent activation in the left amygdala, and the anterior and posterior cingulate cortex in response to intensely fearful faces, and also reduced amplitude in skin conductance fluctuation, which was highly correlated with amygdala activation . Dynamic causal modeling analysis in this data set further showed CBD reduced forward functional connectivity between the amygdala and anterior cingulate cortex .
The patient continued to use cannabis oil for 65 days. The family changed strains of the oil repeatedly, and some were more effective in increasing appetite and alleviating pain than others. The author of the case report suggests that cannabis oil needs to be explored further because there is potential that cannabinoids might show selectivity when attacking cancer cells, thereby reducing the widespread cytotoxic effects of conventional chemotherapeutic agents. So Mechoulam called the Israeli national police and scored five kilos of confiscated Lebanese hashish. He and his research group isolated—and in some cases also synthesized—an array of substances, which he injected separately into rhesus monkeys. Only one had any observable effect. “Normally the rhesus monkey is quite an aggressive individual,” he says. But when injected with this compound, the monkeys became emphatically calm. “Sedated, I would say,” he recalls with a chuckle.
Evidence indicates that CBD is an effective intervention for neuropsychiatric anxiety disorders such as: generalized anxiety disorder, social phobia, panic disorder, PTSD, and OCD. Researchers believe that its anxiolytic effects are principally a result of its affinity for 5-HT1A and CB1 receptors. While further research is warranted regarding long-term use of CBD oil for anxiety, authors note that it does not increase anxiety, has minimal sedation, and is extremely safe when used over a short-term.
Industrial hemp, on the other hand, comes from the engineered Cannabis Sativa strain, which contains only trace concentrations of THC. Although hemp falls under the cannabis category, it’s different from the cannabis plant that’s grown for medicinal or recreational purposes. CBD from industrial hemp doesn’t produce the euphoric buzz that’s commonly associated with intake of marijuana-based CBD oil.
TRPV1 receptor: The TRPV1 (transient receptor potential cation channel subfamily V member 1) receptor is a “vanilloid receptor” associated mostly with the modulation of body temperature and nociception. Cannabidiol is believed to act as a TRPV1 receptor agonist, thereby stimulating the receptor which may reduce sensations of pain and lower inflammation. It is possible that the nociceptive effect associated with TRPV1 agonism also reduces anxiety.

DiPatrizio says, “There may be some benefits outside of improving epilepsy outcomes, but a lot more research is required.” Any research on athletic claims would almost certainly come from the industry; there are more urgent public health CBD topics to investigate than whether it reduces runners’ knee pain. For the foreseeable future, runners interested in CBD’s effectiveness will have to rely on anecdotal, subjective reports.
CBD is a miracle for me! I have anxiety and arthritis really bad. Very painful knee pain and numbness and pain in my shoulders, arms and hands, especially at night. Let's start with the most officially proven medical use of CBD. Earlier this year, the FDA approved the first-ever drug containing CBD, Epidiolex, to treat two rare forms of pediatric epilepsy. To get to that point, the drug's manufacturers had to do a whole lot of randomized, placebo-controlled trials on humans. They had to study how much children could take, what would happen in case of overdose, and any possible side effects that would occur.
Verified CBD is a great example that you shouldn’t judge the book by its cover. Although the information about the company is scant, and the team seems to be completely okay with that, once you engage into an e-mail conversation with them, you will get answers to absolutely any question that bothers you. Besides, when a company has been successfully selling their CBD goods to 50 countries around the world, there must be something in the water.
Yet the DEA has stated unequivocally that it considers CBD to be illegal under the Controlled Substances Act. “CBD derived from the cannabis plant is controlled under Schedule I of the CSA because it is a naturally occurring constituent of marijuana,” Joseph Rannazzisi, the deputy assistant administrator of the DEA, told a congressional panel in June. “While there is ongoing research into a potential medical use of CBD, at this time, CBD has no currently accepted medical use in the USA.” Moreover, DEA spokesman Eduardo Chavez told the New Republic that Medical Marijuana, Inc.’s in-house opinion with regards to CBD has no merit. “The bottom line,” Chavez said, “is the oil is part of the marijuana plant, and the marijuana plant is currently a Schedule I controlled substance under federal law.”
Hemp oil has never been as popular as other marijuana products. With little to no THC, CBD-rich strains of cannabis don’t deliver the pleasant buzz recreational users seek out in marijuana. In the 1970s, however, scientists found that cannabidiol was effective in reducing seizures. The brain’s endocannabinoid system contains receptors that respond to CBD, producing anticonvulsant effects. Being plant-derived and native to the brain’s own chemistry, CBD is therefore one of the most natural options for seizure treatment available today. Still, not many people took interest in CBD until 2013, when a CNN documentary special, Weed, hosted by the network’s chief medical correspondent, Dr. Sanjay Gupta, highlighted CBD’s effectiveness in combating seizures. Since then, demand for hemp oil products has exploded.
Stephanie Kahn, who with her husband, Jeffrey, runs the Takoma Wellness Center, a medical marijuana dispensary in Northwest Washington, says that about half of her 1,200 patients use CBD-rich products. Her dispensary offers several strains of high-CBD cannabis as well as CBD oil, with different ratios of CBD and THC, each of which she recommends for particular conditions. “We get questions about it every day,” she says. “A lot of our patients get relief with this, and a lot of times this works better than pharmaceutical drugs.”
The apparatus used for the polysomnography exams consisted of different devices including electroencephalogram with the international 10–20 system (to rule out the occurrence of epileptic seizures), electrooculogram, electromyogram of chin muscles and upper and lower limbs, nasal pressure cannula, oral thermistor, thoracic and abdominal respiratory inductive plethysmography straps, pulse oximetry, electrocardiogram, and snoring and body position sensors. Video and sound were also recorded during the exam.
Earlier preclinical studies have suggested that the therapeutic effects of CBD might depend on the presence of specific clinical conditions. As an example, Campos et al. (2013) showed that the chronic use of CBD for 2 weeks, while not directly increasing hippocampal neurogenesis, prevented its decrease by unpredictable chronic stress. Thus, the absence of changes in the sleep of healthy volunteers treated with CDB in our study should not be considered as a final indication that CBD could not have positive effects in patients with sleep disorders.
Polysomnography recordings were obtained through a computerized system (BrainNet BNT; LYNX Tecnologia Eletrônica, Rio de Janeiro, Brazil). Sleep stages were recorded in periods of 30 s, according to the criteria established by Rechtschaffen and Kales (1968). The following polysomnographic parameters were evaluated: total sleep time (TST, min), sleep onset latency (min), rapid eye movement (REM) onset latency (min), wake after sleep onset (min), wake after sleep onset index (h), apnea index (h), hypopnea index (h), respiratory disturbance index (RDI, h), sleep efficiency (%), stage 1 sleep (%), stage 2 sleep (%), stage 3 sleep (%), REM (%), lowest saturation (%), and baseline saturation (%).
However, I’m thinking that there may have been some sort of synergistic effect between the CBD and beer. The combination of CBD plus beer worked extremely well for my anxiety – but obviously the beer is not a sustainable nor healthy long-term option. Reflecting on the experience, it’s difficult to determine how well the CBD worked because I was exposed to a lot more anxiety than the first situation.
When all is said and done, CBD oil is of course relatively new compared to traditional medicine, and therefore a patient with sleep trouble should always discuss CBD with a qualified healthcare professional before using. Also, as we have mentioned it’s important to understand that CBD has not been a clinically-verified form of treatment for insomnia.
PPAR agonism: Agonism of PPARs (peroxisome proliferator activated receptors) may have a variety of benefits including: anticancer, neuroprotective (via removal of beta-amyloid plaques), and antipsychotic effects. Cannabidiol bolsters PPAR-alpha signaling and simultaneously decreases inflammation. Although PPAR agonism may not directly foster an anxiolytic effect, it cannot be ruled out as a potential synergistic contributor.
CBD interacts mostly with CB1 receptors which are spread throughout the entire body, but they’re found in the highest concentrations in the immune and nervous systems. The interaction between CBD and endocannabinoid receptors, proteins, and other chemicals in the brain, triggers changes in the activity of hormones, and neurotransmitters throughout the brain and the body.

Worsening of anxiety: Though most research indicates that cannabidiol is likely to decrease anxiety in humans and animal models, contrasting evidence necessitates consideration. A study published in 2012 by ElBatsh et al. examined the effects of CBD administration on rodent behavior and protein expression. Notably, CBD decreased frontal and hippocampal BDNF and reduced TrkB and phosphor-ERK1/2 expression. This suggests that when used frequently, CBD may exacerbate underlying anxiety. For the study, researchers recruited 10 treatment-naïve patients with social anxiety disorders. To determine how CBD influenced neural activity, they utilized functional neuroimaging to assess regional cerebral blood flow at rest with a SPECT scan incorporating an L-ethylcysteinate dimer (ECD) tracer.

That grey area is likely to be clarified soon. Key federal legislators are pushing for industrial hemp legalization to be part of the farm bill that will probably pass in the fall of 2018. In terms of athletics, hemp-derived CBD was removed from the World Anti-Doping Agency’s list of prohibited substances earlier this year. Hemp legalization and more companies targeting athletes should further separate CBD from its cultural association with marijuana.
5-HT1A agonist: 5-HT1A is a subtype of the serotonin receptor, which is important because anxiety and depression can sometimes be treated with medications that target the serotonin system. This is why drug companies developed selective serotonin reuptake inhibitors (SSRIs) like Prozac and Zoloft. SSRIs work by blocking reabsorption of serotonin in the brain, which increases availability of serotonin in the synaptic space. This helps brain cells transmit more serotonin signals, which can reduce anxiety and boost mood in certain cases (although the full biological basis for this is more complicated and not fully understood).
Public speaking: Those who have difficulty with public speaking due to anxiety will likely benefit from CBD. Cannabidiol administration approximately 1.5 hours prior to a simulated public speaking engagement significantly reduced anxiety compared to a placebo. Those with social anxiety were found to derive the most benefit from its administration. However, it is likely that cannabidiol would benefit even those without social phobia if anxiety is experienced during a public speaking task.
Cannabidiol is currently a class B1 controlled drug in New Zealand under the Misuse of Drugs Act. It is also a prescription medicine under the Medicines Act. In 2017 the rules were changed so that anyone wanting to use it could go to the Health Ministry for approval. Prior to this, the only way to obtain a prescription was to seek the personal approval of the Minister of Health.
