The 5-HT1A receptor (5-HT1AR) is an established anxiolytic target. Buspirone and other 5-HT1AR agonists are approved for the treatment of GAD, with fair response rates [50]. In preclinical studies, 5-HT1AR agonists are anxiolytic in animal models of general anxiety [51], prevent the adverse effects of stress [52], and enhance fear extinction [53]. Both pre- and postsynaptic 5-HT1ARs are coupled to various members of the Gi/o protein family. They are expressed on serotonergic neurons in the raphe, where they exert autoinhibitory function, and various other brain areas involved in fear and anxiety [54, 55]. Mechanisms underlying the anxiolytic effects of 5-HT1AR activation are complex, varying between both brain region, and pre- versus postsynaptic locus, and are not fully established [56]. While in vitro studies suggest CBD acts as a direct 5-HT1AR agonist [57], in vivo studies are more consistent with CBD acting as an allosteric modulator, or facilitator of 5-HT1A signaling [58].

Some researchers believe that hippocampal neurogenesis may play a critical role in attenuating symptoms of severe anxiety and/or depression.  Although not all 5-HT1A partial agonists may induce hippocampal neurogenesis, there’s evidence to suggest that cannabidiol does.  A study published in 2013 assessed the anxiolytic effects of CBD in mice exposed to chronic stress.

Microinjection of CBD into the DPAG produced anxiolytic effects in the EPM, VGC, and ETM that were partially mediated by activation of 5-HT1ARs but not by CB1Rs [65, 68]. The bed nucleus of the stria terminalis (BNST) serves as a principal output structure of the amygdaloid complex to coordinate sustained fear responses, relevant to anxiety [93]. Anxiolytic effects of CBD in the EPM and VCT occurred upon microinjection into the BNST, where they depended on 5-HT1AR activation [79], and also upon microinjection into the central nucleus of the amygdala [78]. In the prelimbic cortex, which drives expression of fear responses via connections with the amygdala [94], CBD had more complex effects: in unstressed rats, CBD was anxiogenic in the EPM, partially via 5-HT1AR receptor activation; however, following acute restraint stress, CBD was anxiolytic [87]. Finally, the anxiolytic effects of systemic CBD partially depended on GABAA receptor activation in the EPM model but not in the VCT model [61, 62].
Although nearly all of the published studies found CBD effective for the attenuation of anxiety, there are some notable limitations associated with the research.  Perhaps the most notable limitation is the fact that most CBD studies investigate the effect of acute, single-dose administration.  The problem with this is that it remains unclear as to whether chronic or long-term CBD ingestion maintains therapeutic efficacy.

FAAH inhibitor: The anxiolytic efficacy of CBD may be a result of its ability to act as an enzymatic inhibitor of FAAH (fatty acid amide hydroxylase).  FAAH is an enzyme responsible for metabolizing endocannabinoids such as anandamide, but when inhibited, these endocannabinoid concentrations are increased.  Increased concentrations of endocannabinoids such as anandamide and 2-AG, both of which bind to peripheral CB1/CB2 receptor sites.
It is known that lack of sleep can interfere with certain aspects of cognitive functioning, such as attentional levels (Goel et al., 2009) and PVT, which has a high sensitivity to measure responses that require selective attention (Basner and Dinges, 2011). However, the results of the present study did not show any significant impairment in either the reaction time or number of errors measured by the PVT, suggesting that the attention levels of the volunteers were preserved in the morning after the sleep assessment, regardless of the administration of CBD or placebo. Not having administered the PVT test before CBD and placebo administration does not significantly affect the conclusions once the study does not intend to assess the effect of CBD on baseline vigilance (which would require comparison with baseline PVT results), but to rather evaluate if CBD may be safely administered to patients without affecting their vigilance state overall, such that the patients may safely conduct every-day tasks, like for example driving.

Accordingly, CB1R activation has been suggested as a target for anxiolytic drug development [15, 43, 44]. Proposed agents for enhancing CB1R activation include THC, which is a potent and direct agonist; synthetic CB1R agonists; FAAH inhibitors and other agents that increase eCB availability, as well as nonpsychoactive cannabis phytocannabinoids, including CBD. While CBD has low affinity for the CB1R, it functions as an indirect agonist, potentially via augmentation of CB1R constitutional activity, or via increasing AEA through FAAH inhibition (reviewed in [21]).

Greenish Route's CBD Sleepy Z's ($14; greenishroute.com) contained the most CBD at 30mg, plus 2mg of melatonin, and they came in gummy form, which I enjoyed because I'm 12 at heart. Dynamic causal modeling analysis in this data set further showed CBD reduced forward functional connectivity between the amygdala and anterior cingulate cortex [110].

After fighting the effects of thyroid cancerfor 12 years I wanted to die. Every day. In the primary session, participants were assigned to receive either CBD (400 mg) or a placebo in double-blinded framework.  Thereafter in a second session, participants received the agent that they hadn’t received in the first session; those that received the placebo first received the CBD – and vice-versa.  Measures indicated that after receiving CBD (400 mg), subjective measures of anxiety significantly decreased compared to the placebo.
Just start low. 2 tiny drops (not droppers) for 4 days. If no results, take 4 tiny drops for 4 days. If no results, take 6 tiny drops for 4 days. Keep upping the dose by 2 drops until you find what works for you. I hate that they say “mg” instead of just measuring by drops! So confusing. I take 6 drops for sleep and it works well. Research suggests that cannabinoids, the psychoactive components of unrefined cannabis, regulates neurotransmitter release and produces a wide range of central nervous system effects, including increased pleasure and alteration of memory processes.
Accordingly, CB1R activation has been suggested as a target for anxiolytic drug development [15, 43, 44]. Proposed agents for enhancing CB1R activation include THC, which is a potent and direct agonist; synthetic CB1R agonists; FAAH inhibitors and other agents that increase eCB availability, as well as nonpsychoactive cannabis phytocannabinoids, including CBD. While CBD has low affinity for the CB1R, it functions as an indirect agonist, potentially via augmentation of CB1R constitutional activity, or via increasing AEA through FAAH inhibition (reviewed in [21]).

From this study we can conclude that the acute effects of THC (e.g. increased anxiety) are unfavorable.  Evidence suggests that CBD appears well-tolerated and safe, with no adverse physiological reactions compared to a placebo.  However, since the physiological effects of CBD (600 mg) were of no statistically significant difference from the placebo, it is unclear if CBD elicits any therapeutic effect – even at a seemingly reasonable dose.
Just saw this now. I use the first one on this list. I’ve tried five different brands, some worked better than others. I have found that my sleep is also connected to the food I eat of a night time. So I’ve cut back on sugary, fatty foods. I take a few drops in the evening, always 2 hours before I go to sleep and try to relax. That’s what works for me. Hope it helps
Subjectively, I’d say it took around 15 to 20 minutes before I noticed some sort of an effect; could’ve been shorter or longer (I didn’t have a timer out).  I wasn’t stressed or anxious prior to taking the capsule, so there may not have been as much neurophysiological contrast.  That said, I noticed that I felt psychologically more relaxed and as if I stopped thinking critically about every little thing.

I was expecting CBD to work like a sleeping pill, in that it would put me to sleep almost instantly. It did not do that. And while it didn't seem to have any wild effects on how long it took me to get to sleep, the quality of my pre-sleep bedtime was way more relaxed than that of the week before, when I would lie awake thinking about deadlines, to-dos, and the way I really wish I had responded to that text. (Did I mention I'm Type A?)
