I have been totally off the effexor and all anti-depressants for 2 weeks now. The dizziness is getting much better however my emotions/agitation are horrible. I cry at everything and am extremely crabby/agitated. I realize most of this has to do with the withdrawal. I have another doctor appt beginning of April and she says that if I don't feel better by then I most likely will need to go back on an anti-depressant. For the most part I agree with her. My hopes of proving her wrong as getting slim however. I'd like to know how long it took some of you who have withdrawn from anti-depressants to feel somewhat 'normal' or you knew you had to go back on them? I guess I'm asking if another month is a good amount of time for me to determine what I should do. In some ways I feel like I should start on them again now but I'm not going there yet? BTW, I am in no way feeling suicidal. Mornings seem to be my worst time and by early evenings I feel somewhat better – is this strange too? Based on the existing scientific literature, it is impossible to conclude whether CBD is therapeutically effective as a treatment for anxiety disorders – especially when administered chronically and/or over a long-term.  However, considerable evidence supports the efficacy of CBD when administered acutely for: social phobia, public speaking anxiety, and environmental stress.  Acute administration of CBD appears to improve subjective, physiological, and objective measures of anxiety in stressful situations.

Limited results in healthy subjects also support the efficacy of CBD in acutely enhancing fear extinction, suggesting potential for the treatment of PTSD, or for enhancing cognitive behavioral therapy. Neuroimaging findings provide evidence of neurobiological targets that may underlie CBD’s anxiolytic effects, including reduced amygdala activation and altered medial prefrontal amygdala connectivity, although current findings are limited by small sample sizes, and a lack of independent replication. Further studies are also required to establish whether chronic, in addition to acute CBD dosing is anxiolytic in human. Also, clinical findings are currently limited to SAD, whereas preclinical evidence suggests CBD’s potential to treat multiple symptom domains relevant to GAD, PD, and, particularly, PTSD.

Epidemiological studies of various neuropsychiatric disorders indicate that a higher CBD content in chronically consumed cannabis may protect against adverse effects of THC, including psychotic symptoms, drug cravings, memory loss, and hippocampal gray matter loss [115–118] (reviewed in [119]). Anxiolytic effects of CBD in models of generalized anxiety have been linked to specific receptor mechanisms and brain regions. The midbrain dorsal periaqueductal gray (DPAG) is integral to anxiety, orchestrating autonomic and behavioral responses to threat [91], and DPAG stimulation in humans produces feelings of intense distress and dread [92]. Microinjection of CBD into the DPAG produced anxiolytic effects in the EPM, VGC, and ETM that were partially mediated by activation of 5-HT1ARs but not by CB1Rs [65, 68]. The bed nucleus of the stria terminalis (BNST) serves as a principal output structure of the amygdaloid complex to coordinate sustained fear responses, relevant to anxiety [93]. Anxiolytic effects of CBD in the EPM and VCT occurred upon microinjection into the BNST, where they depended on 5-HT1AR activation [79], and also upon microinjection into the central nucleus of the amygdala [78]. In the prelimbic cortex, which drives expression of fear responses via connections with the amygdala [94], CBD had more complex effects: in unstressed rats, CBD was anxiogenic in the EPM, partially via 5-HT1AR receptor activation; however, following acute restraint stress, CBD was anxiolytic [87]. Finally, the anxiolytic effects of systemic CBD partially depended on GABAA receptor activation in the EPM model but not in the VCT model [61, 62].
The relative representativeness of the small sample size and the use of a single dose of CBD can perhaps be regarded as a limitation of our study, as it does not allow the assessment of the effects of chronic treatment with CBD on sleep. In the study by Chagas et al. (2014b), for example, CBD was chronically administered for 6 weeks to patients with Parkinson’s disease and REM sleep behavior disorder. Since the effects of CBD are biphasic (Zuardi et al., 2017), the use of a single dose also limits the interpretation of the present findings. Moreover, monitoring changes in sleep using a conventional polysomnography presents some intrinsic limitations, as it is insufficient alone to detect drug-induced changes of the sleep EEG. For this purpose, a spectral analysis or a similar procedure is also needed. Conversely, the use of preclinical polysomnography to characterize drug-induced sleep disturbances has been increasingly recommended in the regulatory context (Authier et al., 2016). Finally, it is essential to evaluate the effects of CBD in a larger sample and in individuals diagnosed with sleep disorders in addition to healthy volunteers.

CBD exerts several actions in the brain that explain why it could be effective in treating anxiety.
