If you’re like me, I think you’ll agree with me when I say that lack of sleep really interrupts your life. You can become cranky and irritable, snapping at friends and colleagues while your body is screaming for rest. Maybe you’ve gone down the laundry list of “fixes” — meditation, yoga, alcohol, exercise, an electronics detox before bed, you know the drill— but nothing seems to be working. It can be frustrating beyond measure.
It was actually a bad bout of jet lag after a trip to California that inspired me to finally test out the CBD oil (I'll admit that my weed-based reservations kept me from trying it for the first few months). Knowing that the oil had also helped people with sleep issues, I squeezed one full dropper of the Everyday Plus oil onto my tongue, per the instructions, and waited.
Participants were recruited through advertisements in the local media of the city of Ribeirão Preto, São Paulo, Brazil. Initially, 335 individuals who were interested in participating were evaluated, 265 of whom were excluded in the recruitment interview (which contained questions about clinical data, demographics, psychiatric symptoms, sleep patterns, among others). The remaining 70 participants were asked to keep a sleep log and completed the rating scales on sleep patterns (ESS, Epworth Sleepiness Scale; PSQI, Pittsburgh Sleep Quality Index). After these procedures, 27 participants were considered eligible for the study (Figure ​Figure11) and were randomized into two groups (group 1: placebo – CBD, group 2: CBD – placebo) matched in terms of sex, age, and years of education. To ensure the adequacy of the matching procedure, one participant of each pair had his treatment blindly chosen between the two treatment options available and the other participant (matched to the first one) was assigned to the remaining option.
Most acutely, the discomfort and stiffness I’d felt for months from a meniscus tear (confirmed by MRI) went away. The occasional twinges I had been getting on runs stopped. More significantly, what had been the tear’s near-constant presence in daily life, such as when getting up from sitting, has disappeared. For now I’ve postponed surgery on the tear. It’s impossible to know if CBD was the key factor in any of these changes. Still, at the end of the month I decided to keep taking CBD daily.
Mike, what kind of breast cancer (invasive ductal, I presume)? How many of her lymph nodes were positive? How big was the primary tumor? Reason I ask is that in women with Stage I or IIA tumors that are estrogen-and progesterone-receptor-positive and HER2-negative (ER+/PR+/HER2-) with three or fewer positive lymph nodes, there is a genomic assay test on a sample of the tumor, called OncotypeDX, that will tell doctors whether chemo is necessary or would even work at all. If she lives in a state where medical marijuana is legal, there are doctors who sub-specialize in certifying applications for a medical marijuana card, and in the interim before the card is issued can advise as to the appropriate dose of CBD oil (legal and over-the-counter in all 50 states). Some (though not most) medical oncologists will certify their own patients’ medical marijuana card applications so she need not seek out another doctor; and will advise the appropriate dose for her symptoms. Once she gets her card, the “budtenders” in the licensed dispensaries can advise her as to the right CBD product (with or without THC), strength, and dosage. If she lives in a state where recreational weed is legal, the “budtenders” in the marijuana shops can steer her to the right strength of CBD oil and the right dosage.

The 5-HT1A receptor (5-HT1AR) is an established anxiolytic target. Buspirone and other 5-HT1AR agonists are approved for the treatment of GAD, with fair response rates [50]. In preclinical studies, 5-HT1AR agonists are anxiolytic in animal models of general anxiety [51], prevent the adverse effects of stress [52], and enhance fear extinction [53]. Both pre- and postsynaptic 5-HT1ARs are coupled to various members of the Gi/o protein family. They are expressed on serotonergic neurons in the raphe, where they exert autoinhibitory function, and various other brain areas involved in fear and anxiety [54, 55]. Mechanisms underlying the anxiolytic effects of 5-HT1AR activation are complex, varying between both brain region, and pre- versus postsynaptic locus, and are not fully established [56]. While in vitro studies suggest CBD acts as a direct 5-HT1AR agonist [57], in vivo studies are more consistent with CBD acting as an allosteric modulator, or facilitator of 5-HT1A signaling [58].

Update 12 October 2018 - Cannabis-based medicinal products will be able to be prescribed by specialist doctors for conditions including epilepsy following a change in the law laid in parliament. Nervous system reset: When we experience a freeze-fight-flight response, activity in the autonomic nervous system (ANS) is altered. Anxiety and fear-inducing situations skew activation of the ANS so that the sympathetic branch is more active than the parasympathetic branch.  Administration of CBD has been shown to prevent overactivity in the sympathetic branch via 5-HT1A partial agonism.  This means that administration of CBD prior to or after a highly stressful event may prevent a full-blown nervous breakdown.

In 31 states and the District of Columbia cannabis is legal for some medical uses, and a majority of Americans favor legalization for recreational use. Other countries are rethinking their relationship to pot too. In Uruguay and Canada the drug is legal. Portugal has decriminalized it. Israel and the Netherlands have medical marijuana programs, and in recent years numerous countries have liberalized possession laws.
Reflecting the next morning, I was most surprised by the fact that I never felt "high" in any way—there was never a moment of It's kicking in; I can feel it now like with pain medications or even anti-anxiety drugs. Considering it takes time, consistency, and the right dosage to experience the full effect, I continued taking the oil once a day for the next six days. Here's what went down.
Inhibited liver function: The liver regulates the way different drugs are metabolized within the body; this process is known as hepatic drug metabolism. Higher-than-average doses of CBD oil can slow the hepatic drug metabolism process. As a result, users may not be able to process other drugs as quickly. One study comparing the effects of THC and CBD even found that, while THC increased anxiety by activating the neurotransmitters involved in the "fight or flight" response, CBD actually repressed autonomic arousal—or the nervous system response associated with sudden increases in heart rate or respiration. CBD products that don't contain THC fall outside the scope of the U.S. Drug Enforcement Agency's (DEA) Controlled Substances Act, which means CBD products are legal to sell and consume as long as they don't have THC. That's likely one of the reasons why CBD products, including CBD oil, are becoming more socially acceptable and increasingly popular. In 2016, Forbes reported that CBD products are expected to be a $2.2 billion industry by 2020.
