In a study whose findings have not yet been published, he and a colleague, Daniel Friedman, found that patients receiving CBD in addition to their usual medicines had 39 percent fewer convulsive seizures than patients who remained on their normal drug regimen. Given that the study included only the most treatment-resistant patients, this is an “excellent response,” Devinsky says.

Cannabis sativa, a species of the Cannabis genus of flowering plants, is one of the most frequently used illicit recreational substances in Western culture. The 2 major phyto- cannabinoid constituents with central nervous system activity are THC, responsible for the euphoric and mind-altering effects, and CBD, which lacks these psychoactive effects. Preclinical and clinical studies show CBD possesses a wide range of therapeutic properties, including antipsychotic, analgesic, neuroprotective, anticonvulsant, antiemetic, antioxidant, anti-inflammatory, antiarthritic, and antineoplastic properties (see [11, 12, 16–19] for reviews). A review of potential side effects in humans found that CBD was well tolerated across a wide dose range, up to 1500 mg/day (orally), with no reported psychomotor slowing, negative mood effects, or vital sign abnormalities noted [20].
I’ve been experiencing panic attacks since I was 21 year olds. I am now 48. They are psychologically debilitating with depression repercussions for weeks after. I’ve tried everything – drugs, psychotherapy, meditation, breathing exercises etc. I understand the source of them and psychosis of it – but have never solved them and have pretty much given up hope. I also go through waves of anxiety – which tend to heighten the chance of a panic attack, but the two are not always correlated. Living in CO and being surrounded by CBD discussions, I finally decided to look up and see what CBD might do. Given this thread of info and responses, I’m going to start experimenting with with CBD. The eCB system regulates diverse physiological functions, including caloric energy balance and immune function [28]. The eCB system is also integral to regulation of emotional behavior, being essential to forms of synaptic plasticity that determine learning and response to emotionally salient, particularly highly aversive events [29, 30]. Activation of CB1Rs produces anxiolytic effects in various models of unconditioned fear, relevant to multiple anxiety disorder symptom domains (reviewed in [30–33]). Regarding conditioned fear, the effect of CB1R activation is complex: CB1R activation may enhance or reduce fear expression, depending on brain locus and the eCB ligand [34]; however, CB1R activation potently enhances fear extinction [35], and can prevent fear reconsolidation. Genetic manipulations that impede CB1R activation are anxiogenic [35], and individuals with eCB system gene polymorphisms that reduce eCB tone—for example, FAAH gene polymorphisms—exhibit physiological, psychological, and neuroimaging features consistent with impaired fear regulation [36]. Reduction of AEA–CB1R signaling in the amygdala mediates the anxiogenic effects of corticotropin-releasing hormone [37], and CB1R activation is essential to negative feedback of the neuroendocrine stress response, and protects against the adverse effects of chronic stress [38, 39]. Finally, chronic stress impairs eCB signaling in the hippocampus and amygdala, leading to anxiety [40, 41], and people with PTSD show elevated CB1R availability and reduced peripheral AEA, suggestive of reduced eCB tone [42].


An animal study involving male Wistar rats conducted by Resstel et al. (2009) examined the effect of CBD on restraint stress (RS).  Previous research had demonstrated that the phytocannabinoid cannabidiol (CBD) yielded anxiolytic and antipsychotic properties in animal models.  For this reason, they investigated whether CBD facilitates adaptation to scenarios of inescapable stress and whether this response is mediated by 5-HT1A receptors.

Although nearly all of the published studies found CBD effective for the attenuation of anxiety, there are some notable limitations associated with the research.  Perhaps the most notable limitation is the fact that most CBD studies investigate the effect of acute, single-dose administration.  The problem with this is that it remains unclear as to whether chronic or long-term CBD ingestion maintains therapeutic efficacy.

I was expecting CBD to work like a sleeping pill, in that it would put me to sleep almost instantly. It did not do that. And while it didn't seem to have any wild effects on how long it took me to get to sleep, the quality of my pre-sleep bedtime was way more relaxed than that of the week before, when I would lie awake thinking about deadlines, to-dos, and the way I really wish I had responded to that text. Several studies assessed CBD using contextual fear conditioning. Briefly, this paradigm involves pairing a neutral context, the conditioned stimulus (CS), with an aversive unconditioned stimulus (US), a mild foot shock. After repeated pairings, the subject learns that the CS predicts the US, and subsequent CS presentation elicits freezing and other physiological responses. Systemic administration of CBD prior to CS re-exposure reduced conditioned cardiovascular responses [63], an effect reproduced by microinjection of CBD into the BNST, and partially mediated by 5-HT1AR activation [79]. Similarly, CBD in the prelimbic cortex reduced conditioned freezing [70], an effect prevented by 5-HT1AR blockade [87]. By contrast, CBD microinjection in the infralimbic cortex enhanced conditioned freezing [70]. Finally, El Batsh et al. [80] reported that repeated CBD doses over 21 days, that is chronic as opposed to acute treatment, facilitated conditioned freezing. In this study, CBD was administered prior to conditioning rather than prior to re-exposure as in acute studies, thus further directly comparable studies are required.

Are you willing to share what you're anxiety is about? I had panic and anxiety attacks for years and used Ativan and later Lorazepam for them, especially when I had to fly in planes. About 6 years ago I started taking Citalopram antidepressant, and all my anxiety/panic attacks went away. I only had it again recently during my withdrawal process. I know my relief from anxiety is from the Citalopram. I even did 7 zip lines in Costa Rica a little over a year ago. My flights there, to Panama and back were anxiety free. That has been so incredible for me! I love traveling now.
CBD is available in oils, or it can be added to creams, ointments and beauty products. It can also be used in a vape pen or even consumed through food like CBD gummies. Joel Greengrass, CEO of Theramu, a company that creates non-THC CBD oil, said he has observed a huge increase in interest and popularity of CBD for skin and wellness complaints, as well as for the treatment of a range of anxiety conditions.
Overall, preclinical evidence supports systemic CBD as an acute treatment of GAD, SAD, PD, OCD, and PTSD, and suggests that CBD has the advantage of not producing anxiogenic effects at higher dose, as distinct from other agents that enhance CB1R activation. In particular, results show potential for the treatment of multiple PTSD symptom domains, including reducing arousal and avoidance, preventing the long-term adverse effects of stress, as well as enhancing the extinction and blocking the reconsolidation of persistent fear memories.
One of the biggest players in this new industry is Medical Marijuana, Inc., a company formed in 2009 that operates out of Poway, California, just north of San Diego. It has played a leading role in the so-called Green Rush, as businesses have moved quickly to capitalize on the gradual legalization of marijuana for medical and recreational purposes by states across the country. The company’s spokesman, Andrew Hard, boasted that Medical Marijuana, Inc., “created the CBD industry and was first to market with CBD products.” Through its various subsidiaries, Medical Marijuana, Inc. sells some of the most recognizable products on the cannabis market— everything from Cibaderm CBD-infused shampoo to CanChew chewing gum. In 2014, the company generated $14.5 million in revenue.

Preliminary research indicates that cannabidiol may reduce adverse effects of THC, particularly those causing intoxication and sedation, but only at high doses.[24] Safety studies of cannabidiol showed it is well-tolerated, but may cause tiredness, diarrhea, or changes in appetite as common adverse effects.[25] Epidiolex documentation lists sleepiness, insomnia and poor quality sleep, decreased appetite, diarrhea, and fatigue.[3]


Tolerance: It is possible that someone who uses CBD oil often could become tolerant to its effects. This is because no drug is capable of bypassing the endogenous homeostatic mechanisms of the human body.  If something were capable of doing so, people could remain on an anxiolytic and/or antidepressant for an indefinite period of time without any decreased efficacy.  Unfortunately, it is likely that if used too frequently, tolerance will ensue and an individual will require greater doses to maintain a therapeutically anxiolytic effect.
PPAR agonism: Agonism of PPARs (peroxisome proliferator activated receptors) may have a variety of benefits including: anticancer, neuroprotective (via removal of beta-amyloid plaques), and antipsychotic effects.  Cannabidiol bolsters PPAR-alpha signaling and simultaneously decreases inflammation.  Although PPAR agonism may not directly foster an anxiolytic effect, it cannot be ruled out as a potential synergistic contributor.

Laboratory evidence indicated that cannabidiol may reduce THC clearance, increasing plasma concentrations which may raise THC availability to receptors and enhance its effect in a dose-dependent manner.[26][27] In vitro, cannabidiol inhibited receptors affecting the activity of voltage-dependent sodium and potassium channels, which may affect neural activity.[28] A small clinical trial reported that CBD partially inhibited the CYP2C-catalyzed hydroxylation of THC to 11-OH-THC.[29]
Sample sizes: As was already mentioned, the sample sizes used to test the effects of CBD for anxiety were relatively small-scale. Although the results from these small-scale studies may be accurate, larger-scale trials (with larger sample sizes) are warranted to confirm preliminary outcomes.  A therapeutic effect found in just 10-20 patients may not hold up in a group of several hundred.
Cross-sectional studies have found a direct correlation between more severe PTSD symptomatology and increased motivation to use cannabis for coping purposes, especially among patients with difficulties in emotional regulation or stress tolerance. When using cannabis treatment, military veterans with PTSD reported reduced anxiety and insomnia and improved coping ability. (5)

After months of visiting doctors and sitting through tests like a human lab rat, it was determined that there was a slight anomaly in the anatomy of my temporal lobe—the part of the brain that controls hearing, speech, and auditory comprehension—which explains why every time I have a seizure, I suddenly don’t understand the English language. Epilepsy can’t be cured, so the only course of action available for me was to take a medication every day for the rest of my life. My neurologist prescribed a few different anti-convulsant medications, but they all made me feel tired, depressed, slow, and unlike myself—until finally, I found one that was slightly better than the rest.
Blake Pearson, founder of GreenlyMed and a practicing doctor in Ontario, Canada, specializes in cannabinoid medicine and said he has personally seen patients who have lowered their intake of prescription medications or reduced the negative side effects of taking other medications. However, Pearson would like to see more robust research, including random controlled trials.
For the study, researchers recruited 8 volunteers and administered the following: THC (0.5 mg/kg), CBD (1 mg/kg), CBD/THC mix OR Valium (10 mg) or placebo (serving as controls).  The volunteers each received the combinations in an order different from the others.  Researchers were able to verify that CBD inhibited anxiety as induced by THC, but physiological data revealed it was not a result of direct THC inhibition.
Even as the research proceeds, thousands of people are using CBD as medicine. A British pharmaceutical company, GW Pharma, has developed two CBD drugs: Sativex, which contains a 1-to-1 ratio of CBD and THC, and Epidiolex, which is pure CBD. The former is prescribed for the painful muscle spasms that occur in multiple sclerosis, while the latter is aimed at childhood seizures. Sativex is not available in the United States, but it is approved in 29 other countries, including Canada, England and Israel.

The definitions of hemp and marijuana can get pretty confusing, but for basic purposes, marijuana contains high levels of THC, and hemp contains low levels of THC. The ratios of CBD to THC in hemp oil can vary, depending on the product and the specific plant the oil was extracted from. CBD oil, a concentrated version of the cannabidiol compound, is typically derived from hemp but can be extracted from marijuana as well. CBD oil products on the market have varying levels of CBD and THC. Many have little to no THC, while some contain small amounts.

“DEA will continue to support sound and scientific research that promotes legitimate therapeutic uses for FDA-approved constituent components of cannabis, consistent with federal law,” acting DEA administrator Uttam Dhillon said in a press release. “DEA is committed to continuing to work with our federal partners to seek ways to make the process for research more efficient and effective.”
Fortunately, the party stopped at my friends and most people left, leaving us to just hang out and chat for a bit (which is what I wanted).  At some point during the night I was cajoled into drinking a couple of beers (not something I’d normally agree to), but was trying to live it up for once.  Comparatively, I’d say that the beer helped more than the CBD in terms of taking the anxious “edge off.”

One study comparing the effects of THC and CBD even found that, while THC increased anxiety by activating the neurotransmitters involved in the "fight or flight" response, CBD actually repressed autonomic arousal—or the nervous system response associated with sudden increases in heart rate or respiration. In other words, CBD is ideal for people looking to relax and unwind—not get out of their minds.
For instance, some people report a sense of calm and peace; others report increased anxiety levels and unpleasant sensations. The intensity of these symptoms will largely depend on an individual’s body composition. In addition, marijuana strains feature different levels of oil concentration that also determines the intensity of the outcomes that a user feels after consumption. Some strains are recommended to produce less profound symptoms and reactions.
As it turns out, healthy sleep-wake cycles are extremely dependent on our state of “alertness” during the day. If you are a victim of insomnia, for example, you (along with millions of other individuals) are likely drowsy, fatigued, and generally “out-of-sorts” during the afternoon. As you might imagine, this wreaks havoc on your sleep-wake cycle as it makes it nearly impossible to enter and maintain the non-REM sleep that you need at night.
I had come to meet Dr. Angel Hernandez, the director of the hospital’s pediatric epilepsy program. A trail of wall-mounted signs led me to the pediatric neurology ward, a bright and airy space with flat-screen TVs running cartoons nonstop. Decorative kites were strung up in the corridors, and rainbow curtains lined the windows. Some of the kids in the waiting area that morning were alert and awake, others groggy. Some were strapped into special strollers designed for children with mobility problems, and some had shaven heads and healing scars. Hernandez came out to greet me, and I was surprised he recognized me after what felt like a very long time. He had diagnosed me with epilepsy in 2004 and treated me for several years.
To this point, CBD oil has existed in a kind of liminal space— at once an illegal drug, a legal medication, and some kind of “dietary” supplement. It’s possible this could change in the coming years, however. GW Pharmaceuticals, a U.K.-based firm, has developed a “pure CBD” medication called Epidiolex that has shown promising test results. It is currently on a fast-track to receive FDA clearance. For some patients, Epidiolex could be a miracle cure. This summer, in Wired magazine, writer Fred Vogelstein chronicled his family’s own struggles to find an effective treatment for his son’s epilepsy—including experiments with hemp oil— and the immense hurdles they overcame to gain access to Epidiolex prior to its FDA approval. The drug could be for sale on pharmacy shelves in the near future, though exactly how near is hard to say.
Their CBD oil is high-quality due to their unique CO2 extraction process which contains no pesticides, solvents, herbicides or chemical fertilizers. Furthermore, they have all their products tested by third-party laboratories. According to customers, their star product has been great for sleep disorders, anxiety, pain relief and for taking on stress, although it’s important to note that not everyone may experience the same results.
Throughout recent years, cannabis oil has been utilized as a viable treatment for anxiety and depression. Moreover, it is continually being looked into by researchers. Truth be told, the impacts of CBD on anxiety is at present thought to be a standout amongst the most captivating and well-funded sectors of current cannabis research; if development proceeds in the way that it has in the course of the past years, at that point we will unquestionably expand exceptionally compelling means by which oils for anxiety and depression can be utilized as a viable treatment.
But it’s Guzmán’s brain tumor research that has captured headlines—and the interest of pharmaceutical companies. With that said, I'm definitely intrigued enough by the subtle effects to continue taking the oil and possibly even to up the dosage to the recommended two full droppers of the 30mL bottle per day for a week or so. Plus, I take comfort in knowing that it's an all-natural treatment for anxiety that's responsibly grown on family farms in Colorado. Something that's safe, legal, requires no prescription, and makes me less anxious, less scatterbrained, and more focused? I'm definitely on board.
However, the 2014 federal farm bill allowed for “research” cultivation and marketing of industrial hemp if those activities aren’t in violation of state laws. Only four states—Idaho, North Dakota, Nebraska, and Kansas—have strict no-CBD laws. Since 2014, there has been little to no federal enforcement against commercial hemp products. The upshot: Functionally, hemp-derived CBD products are safe for interstate commerce.
Some users speculate about appropriate dosages or methods of application—including whether or not a small amount of THC boosts CBD’s effects, or whether different methods of administration lead to quicker or more significant effects. Some CBD producers also claim that it has a cumulative effect, and so needs to be used regularly to produce a benefit. But Grant says it’s tough to say at this point exactly how people should (or shouldn’t) be using CBD.
Basically, CBD is a 100% natural chemical that’s found in the marijuana plant. It is what’s referred to as a “phytocannabinoid,” which means it belongs to a class of molecules that interact with endocannabinoid receptors in the human body. These receptors belong to the body’s endocannabinoid system, or ECS, which is responsible for essentially all of our homeostatic functions.
A study published in 1993 by Zuardi et al. attempted to elucidate the effects of ipsapirone and cannabidiol among humans exposed to an experimental anxiety task.  For the study, researchers recruited 40 healthy volunteers that were assigned to a simulated public speaking (SPS) test – designed to mimic the effects of public speaking.  The 40 participants were divided into 4 groups of 10 – each of which was assigned randomly to receive: CBD (300 mg), Valium (10 mg), ipsapirone (5 mg), or a placebo.
Prescription medicine (Schedule 4) for therapeutic use containing 2 per cent (2.0%) or less of other cannabinoids commonly found in cannabis (such as ∆9-THC). A schedule 4 drug under the SUSMP is Prescription Only Medicine, or Prescription Animal Remedy – Substances, the use or supply of which should be by or on the order of persons permitted by State or Territory legislation to prescribe and should be available from a pharmacist on prescription.[71]
While normally I'd be slightly tripped up by little things like an overly crowded subway car or a full inbox at work, the CBD oil seems to have taken the edge off of my anxiety a bit. Rather than overthinking a sternly worded email or analyzing a social interaction, I've found it easier to recognize the irrationality of these thoughts and actually let them go (instead of ruminating on the situation). In some ways, I feel more like myself. With that said, I've still experienced some social anxiety when meeting new groups of people—I'd be interested to see what taking the full recommended dose would do.
Following cloning of the endogenous receptor for THC, namely the CB1R, endogenous CB1R ligands, or “endocannabinoids” (eCBs) were discovered, namely anandamide (AEA) and 2-arachidonoylglycerol (reviewed in [22]). The CB1R is an inhibitory Gi/o protein-coupled receptor that is mainly localized to nerve terminals, and is expressed on both γ-aminobutryic acid-ergic and glutamatergic neurons. eCBs are fatty acid derivatives that are synthesized on demand in response to neuronal depolarization and Ca2+ influx, via cleavage of membrane phospholipids. The primary mechanism by which eCBs regulate synaptic function is retrograde signaling, wherein eCBs produced by depolarization of the postsynaptic neuron activate presynaptic CB1Rs, leading to inhibition of neurotransmitter release [23]. The “eCB system” includes AEA and 2-arachidonoylglycerol; their respective degradative enzymes fatty acid amide hydroxylase (FAAH) and monoacylglycerol lipase; the CB1R and related CB2 receptor (the latter expressed mainly in the periphery); as well as several other receptors activated by eCBs, including the TRPV1 receptor, peroxisome proliferator-activated receptor-γ, and G protein-coupled 55 receptor, which functionally interact with CB1R signaling (reviewed in [21, 24]). Interactions with the TRPV1 receptor, in particular, appear to be critical in regulating the extent to which eCB release leads to inhibition or facilitation of presynaptic neurotransmitter release [25]. The TRPV1 receptor is a postsynaptic cation channel that underlies sensation of noxious heat in the periphery, with capsacin (hot chili) as an exogenous ligand. TRPV1 receptors are also expressed in the brain, including the amygdala, periaqueductal grey, hippocampus, and other areas [26, 27].

To determine the effects of each substance, physiological measures were collected along with symptom ratings approximately 1, 2, and 3 hours post-administration.  Post-trial assessment of physiological measures indicated that compared to placebo and CBD, administration of THC caused anxiety, dysphoria, positive psychotic symptoms, neurophysiological sedation, and elevated heart rate.  Strikingly, there appeared to be no significant differences between CBD and placebo on physiological or symptomatic measures.

There are two types of cannabinoid receptors. CB1 receptors are located in the central and peripheral nervous system and are credited with creating homeostasis with health and disease. CB2 receptors are located in the immune system, gastrointestinal system, and the brain. In a 2001 study, researchers from Vanderbilt conducted a study on mice to search for CB1 receptors in the central amygdala — an area of the brain associated with anxiety and stress responses. They found the presence of receptors in the mouse brain and furthermore, discovered that when endocannabinoids interacted with them that the excitability of these brain cells decreased. Further studies are needed to prove this finding.
While CBD predominantly has acute anxiolytic effects, some species discrepancies are apparent. In addition, effects may be contingent on prior stress and vary according to brain region. A notable contrast between CBD and other agents that target the eCB system, including THC, direct CB1R agonists and FAAH inhibitors, is a lack of anxiogenic effects at a higher dose. Further receptor-specific studies may elucidate the receptor specific basis of this distinct dose response profile. Further studies are also required to establish the efficacy of CBD when administered in chronic dosing, as relatively few relevant studies exist, with mixed results, including both anxiolytic and anxiogenic outcomes.
Accordingly, CB1R activation has been suggested as a target for anxiolytic drug development [15, 43, 44]. Proposed agents for enhancing CB1R activation include THC, which is a potent and direct agonist; synthetic CB1R agonists; FAAH inhibitors and other agents that increase eCB availability, as well as nonpsychoactive cannabis phytocannabinoids, including CBD. While CBD has low affinity for the CB1R, it functions as an indirect agonist, potentially via augmentation of CB1R constitutional activity, or via increasing AEA through FAAH inhibition (reviewed in [21]).
Although not as abundant as THC cannabinoid content, cannabidiol accounts for approximately 40% of all cannabinoids within cannabis extract.  Unlike THC, cannabidiol is non-psychoactive and isn’t typically ingested with the intent to attain any sort of psychological euphoria.  That said, the medicinal properties associated with cannabidiol (often administered in the format of “CBD oil”) are thought to far exceed those of THC.
Even as the research proceeds, thousands of people are using CBD as medicine. A British pharmaceutical company, GW Pharma, has developed two CBD drugs: Sativex, which contains a 1-to-1 ratio of CBD and THC, and Epidiolex, which is pure CBD. The former is prescribed for the painful muscle spasms that occur in multiple sclerosis, while the latter is aimed at childhood seizures. Sativex is not available in the United States, but it is approved in 29 other countries, including Canada, England and Israel.

It's the Wild West out there. Without any federal regulatory body checking labels, consumers have very little way of knowing what they're buying when they purchase CBD oil. Bonn-Miller co-authored a study that found that 26 percent of CBD products on the market contained less CBD than their label claimed. So the amount you need for an effective dose could vary drastically, not just from product to product, but from bottle to bottle of the same product.
It is known that lack of sleep can interfere with certain aspects of cognitive functioning, such as attentional levels (Goel et al., 2009) and PVT, which has a high sensitivity to measure responses that require selective attention (Basner and Dinges, 2011). However, the results of the present study did not show any significant impairment in either the reaction time or number of errors measured by the PVT, suggesting that the attention levels of the volunteers were preserved in the morning after the sleep assessment, regardless of the administration of CBD or placebo. Not having administered the PVT test before CBD and placebo administration does not significantly affect the conclusions once the study does not intend to assess the effect of CBD on baseline vigilance (which would require comparison with baseline PVT results), but to rather evaluate if CBD may be safely administered to patients without affecting their vigilance state overall, such that the patients may safely conduct every-day tasks, like for example driving.

It’s important to note that each state has its own individual laws on possession limits. Many states now have their own laws on the books for CBD oil specifically. Tennessee, for example, has made cannabis oil legal if it’s derived from hemp rather than marijuana. As Professor Elliot Altman of the Botanical Medical Research Center at Middle Tennessee State University, explains, “The legal definition is hemp is less than point three percent THC which is the psychotropic agent. Marijuana is point three percent or greater.” (14)


DiPatrizio says, “There may be some benefits outside of improving epilepsy outcomes, but a lot more research is required.” Any research on athletic claims would almost certainly come from the industry; there are more urgent public health CBD topics to investigate than whether it reduces runners’ knee pain. For the foreseeable future, runners interested in CBD’s effectiveness will have to rely on anecdotal, subjective reports.
