Cannabidiol (CBD), a Cannabis sativa constituent, is a pharmacologically broad-spectrum drug that in recent years has drawn increasing interest as a treatment for a range of neuropsychiatric disorders. The purpose of the current review is to determine CBD’s potential as a treatment for anxiety-related disorders, by assessing evidence from preclinical, human experimental, clinical, and epidemiological studies. We found that existing preclinical evidence strongly supports CBD as a treatment for generalized anxiety disorder, panic disorder, social anxiety disorder, obsessive–compulsive disorder, and post-traumatic stress disorder when administered acutely; however, few studies have investigated chronic CBD dosing. Likewise, evidence from human studies supports an anxiolytic role of CBD, but is currently limited to acute dosing, also with few studies in clinical populations. Overall, current evidence indicates CBD has considerable potential as a treatment for multiple anxiety disorders, with need for further study of chronic and therapeutic effects in relevant clinical populations.

Even without changes at the federal level, there are steps that states could take on their own to make the CBD market safer. States with broad marijuana legality or CBD-only measures could mandate the calibration and regulation of testing labs, and use them to conduct safety testing. They could fund research into the benefits, dosing, and drug interactions of CBD through their public university systems. Medical boards could redouble efforts to educate physicians in what research exists regarding medical marijuana in all its incarnations, so that doctors are prepared to prescribe and manage these medications as they become available.
Hey Linda. Sorry to hear you are struggling with sleep. I know how frustrating this can be. As I’m not a medical professional, I cannot give you advice on dosage for CBD. The Mayo Clinic used to have a dosage guidelines page but they have since taken it down. The dosage they had listed which could potentially help with sleep was 40 mg to 160 mg of CBD. I recommend you let your prescribing physician know you are using CBD alongside the Lunesta.

The go/no-go task measures response inhibition, and is associated with activation of medial prefrontal, dorsolateral prefrontal, and parietal areas [111]. Response activation is diminished in PTSD and other anxiety disorders, and increased activation predicts response to treatment [112]. CBD produced no changes in predicted areas (relative to placebo) but reduced activation in the left insula, superior temporal gyrus, and transverse temporal gyrus. The fearful faces task activates the amygdala, and other medial temporal areas involved in emotion processing, and heightened amygdala response activation has been reported in anxiety disorders, including GAD and PTSD [113, 114]. CBD attenuated blood-oxygen-level dependent activation in the left amygdala, and the anterior and posterior cingulate cortex in response to intensely fearful faces, and also reduced amplitude in skin conductance fluctuation, which was highly correlated with amygdala activation [109]. Dynamic causal modeling analysis in this data set further showed CBD reduced forward functional connectivity between the amygdala and anterior cingulate cortex [110].
The cost of treatment varies: Depending on the dispensary and the dosage, it can range from around $100 a month to more than $1,000. Despite the cost, which is not covered by insurance, CBD medicines are drawing great interest for children with severe, intractable epilepsy. California and Colorado, which were among the first states to legalize medical marijuana, have become hot spots for such patients. Before other states legalized medicinal CBD use, some families moved to these states so they could have access to the compound.

Although the 5-HT1A receptor partial agonism is thought to facilitate a majority of CBD’s anxiolytic effects – hippocampal neurogenesis, opioidergic modulation, and CB1/CB2 inverse agonism likely also contribute.  Lesser researched mechanisms of CBD that could also decrease anxiety include: FAAH inhibition, adenosine reuptake inhibition, GPR55 antagoism, intracellular calcium (Ca2+) increases, and PPAR agonism.  Of these mechanisms, inhibition of FAAH may be most significant in regards to anxiety attenuation.

Hello and thanks for your comment. I’m sorry to hear you haven’t had any success with the CBD for helping you sleep. Most people find that CBD helps them get to sleep better if taken within 1 hour of going to bed. How much have you been taking? As long as you are comfortable with it, you may want to increase the dose a little bit to see if that has a better effect for sleep.
Increased anxiety: Rodents administered cannabidiol daily for 14 days exhibited anxiogenic behaviors. In other words, the cannabidiol may increase anxiety when used too regularly.  Although this effect cannot be confirmed in humans, it is logical to assume that a person’s neurophysiology will adapt to the effects of CBD when used regularly, possibly blunting its efficacy.
Hash oil is an extracted cannabis product that may utilize any part of the plant. Ideally, the final product will not contain any residual traces of solvents. It is generally thought to be indistinct from traditional hashish according to the 1961 UN Single Convention on Narcotic Drugs (Schedule I and IV) as it is "the separated resin, whether crude or purified, obtained from the cannabis plant".

Indeed, hemp oil products have grown out of a market largely devoid of regulations or safety protocols. The state of the CBD industry harks back to the age of elixirs and potions hawked from covered wagons to the awed denizens of pioneer towns. There is no regulatory body screening products for pesticides, heavy metals, solvent residues, and other dangerous contaminants. Long-term outcomes: There are zero long-term studies investigating the safety, efficacy, and long-term effects of CBD as a treatment for anxiety. Data from animal model studies suggests that chronic CBD usage could yield deleterious epigenetic and/or neuropsychiatric effects. However, it is unclear as to whether administration of CBD at a normative (non-chronic) frequency would maintain therapeutic efficacy over a long-term.

The good news is that most of the official research done on CBD oil has shown that there are very few negative side effects from using it. However, CBD is not without some side effects. Most notably, in the clinical studies for epilepsy, sedation was one of the more common side effects. Decreased appetite and diarrhea were also seen in some patients. Interactions: CBD, especially when ingested at high doses, may interact with other pharmacological agents, including prescription drugs. Cannabidiol inhibits CYP450 isoenzymes in the liver which means it may be contraindicated with drugs like Warfarin.  Researchers should attempt to understand the full-spectrum of CBD interactions and refine usage guidelines for those taking other medications.

Genetic manipulations that impede CB1R activation are anxiogenic [35], and individuals with eCB system gene polymorphisms that reduce eCB tone—for example, FAAH gene polymorphisms—exhibit physiological, psychological, and neuroimaging features consistent with impaired fear regulation [36]. Reduction of AEA–CB1R signaling in the amygdala mediates the anxiogenic effects of corticotropin-releasing hormone [37], and CB1R activation is essential to negative feedback of the neuroendocrine stress response, and protects against the adverse effects of chronic stress [38, 39]. Finally, chronic stress impairs eCB signaling in the hippocampus and amygdala, leading to anxiety [40, 41], and people with PTSD show elevated CB1R availability and reduced peripheral AEA, suggestive of reduced eCB tone [42].
Subjectively, I’d say it took around 15 to 20 minutes before I noticed some sort of an effect; could’ve been shorter or longer (I didn’t have a timer out).  I wasn’t stressed or anxious prior to taking the capsule, so there may not have been as much neurophysiological contrast.  That said, I noticed that I felt psychologically more relaxed and as if I stopped thinking critically about every little thing.
